Recently, doctors at the Center for Nephrology, Urology and Dialysis, together with multiple departments across Bach Mai Hospital, achieved a remarkable medical breakthrough: saving the life of a 17-year-old high school student from the brink of death by successfully identifying and treating one of the rarest diseases in the world.

Breath-Holding Moments: When Hope Seemed to Fade

Patient T.N.H. was admitted in a critical condition. Within just 24 hours, the previously healthy young girl was struck by a violent “medical storm.” Her condition deteriorated rapidly with severe hypoalbuminemia (23 g/L), profound anemia (Hb 72 g/L), and platelet counts dropping to a “life-threatening” level (<10 G/L).

Upon admission, the patient was in critical condition with a poor prognosis

Her lungs filled with blood due to diffuse alveolar hemorrhage, causing her oxygen saturation (SpO₂) to fall below 80% despite high-flow oxygen therapy. At the same time, she developed acute heart failure, with blood pressure dropping below 70/40 mmHg and episodes of paroxysmal tachycardia reaching 180 beats per minute. Both kidneys completely “shut down,” leaving her anuric.

As if that were not enough, the patient also suffered splenic infarction and pneumonia caused by multidrug-resistant Klebsiella aerogenes. On the background of newly diagnosed systemic lupus erythematosus (SLE) discovered during this admission, the rapidly progressing multi-organ failure pushed her vital signs to the edge of hopelessness.

Yet, amid the constant sounds of ventilators and dialysis machines, the doctors of Bach Mai Hospital fought relentlessly to save her life, determined not to let the young girl remain “forever 17.”

A Medical Turning Point: A Bold Decision Against the Conventional Path

Dr. Nghiem Trung Dung, Director of the Center for Nephrology, Urology and Dialysis, explained: “Normally, when a lupus patient develops severe kidney failure, physicians immediately think of lupus nephritis-often class IV, possibly with crescentic lesions-and treatment is quickly initiated with strong, sometimes very aggressive, immunosuppressive therapy.

However, such an approach would place the patient at even greater risk of infection, especially when she was already suffering from severe pneumonia caused by multidrug-resistant Klebsiella. If we had followed that conventional line of thinking, her fate might have been very different.”

After carefully analyzing clinical findings, laboratory data, and drawing upon their extensive experience, the medical team rejected the common diagnoses and arrived at a groundbreaking conclusion: the patient was suffering from Catastrophic Antiphospholipid Syndrome (CAPS) secondary to systemic lupus erythematosus.

An intense yet deeply engaging consultation between clinical physicians and pathologists

CAPS is an extremely rare variant of Antiphospholipid Syndrome (APS). Globally, only about 600 cases have been reported from all causes, with a mortality rate exceeding 50%. Unlike typical lupus kidney damage, this condition produces countless microscopic blood clots that block the vascular system, effectively starving organs of oxygen and leading to rapid multi-organ failure.

Subsequent kidney pathology confirmed that the patient had Class II lupus nephritis, indicating only mild kidney involvement—far from requiring aggressive immunosuppressive therapy, especially in the context of a severe ongoing infection.

A Life-Saving Decision That Turned the Tide

Correctly identifying the disease mechanism allowed the treatment team to make a life-or-death decision: emergency plasma exchange (PLEX) to remove the pathogenic antibodies from the bloodstream.

This proved to be the critical “golden decision”, halting the cascade of multi-organ failure and saving the young girl’s life just in time. At the same time, avoiding immediate aggressive immunosuppression enabled her body to better combat the multidrug-resistant bacterial infection that was directly threatening her life.

The success of this case clearly demonstrates the effectiveness of the Multidisciplinary Team (MDT) model at Bach Mai Hospital. Seamless collaboration between the departments of Nephrology and Dialysis, Intensive Care, Hematology, and various diagnostic units created a comprehensive and coordinated treatment pathway.

Several advanced medical techniques were implemented simultaneously, including:

Continuous Renal Replacement Therapy (CRRT)
Plasma Exchange (PLEX)
Carefully adjusted immunosuppressive therapy
Targeted antibiotic treatment for multidrug-resistant bacterial infections
Following intensive treatment, the patient’s condition improved dramatically. She has now returned to a healthy life with only minimal immunosuppressive therapy and virtually no impact on her quality of life.

Twelve weeks later, follow-up tests were conducted to confirm the diagnosis of antiphospholipid syndrome. The patient continues anticoagulation therapy in accordance with medical guidelines to ensure long-term prognosis, including considerations related to future reproductive health.

Alongside the tireless efforts of the medical team was a deeply moving story of love and devotion. H.’s stepmother remained by her side day and night, caring for her with unwavering dedication despite significant financial hardship.

Her tearful plea-“Please do everything possible to save my child”-became a priceless source of emotional strength, giving the young girl the courage to fight through the darkest moments. That extraordinary mother-daughter bond also became a powerful motivation for the doctors throughout the entire treatment journey.

Witnessing the remarkable recovery of the young patient after three months of intensive treatment, Assoc.
